LOCATION: JOHN H. STROGER HOSPITAL/INTEGRATED CARE DEPARTMENT
SHIFT: 8:30 AM - 4:30 PM
Using evidenced based practices, the Manager of Transitional Care is responsible for developing the systems, processes and workflows for patients who are moving between care settings. These transitions may include patients leaving the acute medical or psychiatric facility, patients being discharged from Cermak Health Services or those being discharged from skilled/custodial nursing facilities. The Manager of Transitional Care manages a team composed of Care Coordinators and Community Health Workers who work to improve the outcomes for patients at risk due to care transitions.
Bachelor's degree from an accredited college or university. (Must provide official transcripts at time of interview).
Must be licensed as a Registered Professional Nurse in the State of Illinois.
A minimum of two (2) years of work experience in a health care setting such as home care, physician's office/ambulatory care, acute care is required.
A minimum of two (2) years of supervisory or management work experience is required.
A minimum of one (1) year of care coordination work experience is required.
A minimum of one (1) year of work experience pertaining to transition related activities is required.
Intermediate proficiency using Microsoft applications is required.
Must possess a valid Illinois driver's license and mandatory vehicle insurance as required in the State of Illinois. (Must provide Illinois driver's license and vehicle insurance at time of interview)

A copy of a valid State ID Card or Driver's License which identifies the holder of the ID as a Veteran, may also be attached to the application at time of filing.
