Swedish Medical Center is looking for an InpatientPharmacist \(0\.8 FTE, variable shifts\) to work at Swedish First Hill in Seattle, WA\.
We are seeking a detail focused, caring, and thoughtful individual in Pharmacy care, promoting the goals and upholding the ideals of Swedish Medical Center in providing excellent care and experience for our diverse patient population\. This position is a member of a multi\-disciplinary team and is responsible for assuring safe, efficient, cost effective drug therapy for all patients\.
Schedule: Days w/ rotating evenings and every other weekend\.
In this position you will:
Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability and makes recommendations for therapy changes to the providers and facility staff as appropriate\.
Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication \(including over\-the\-counter and home remedies\)\.
Collaborates with the healthcare team to provide clinical and technical support as needed\.
Documents all communication with physicians, nurses, other members of the healthcare team and patients and their family members in the electronic medical record\.
Provide pharmaceutical care that includes cost conscious therapeutic drug monitoring, pharmacokinetic evaluations, and documentation of patient medical records\.
Respond to pharmaceutical inquiries from patients, medical personnel, and pharmacy staff\.
Oversee activities and assign projects to resident pharmacist, interns and other pharmacy personnel\.
Responsible for, handling incoming physician calls, logging data into the computer, and Drug Utilization Review\.

Performs other tasks as assigned\.
Requiredqualifications for this position include:
Graduate of an accredited school of pharmacy
Current license to practice as registered pharmacist in the State of Washington
Ability to accurately check the compounding of IV admixtures, TPN and other parental drugs and chemotherapy drugs
Ability to follow collaborative practice agreement \(CPA\) and pharmacy guidelines and protocols for appropriate monitoring and dosing of antimicrobial agents, anticoagulation, Glycemic control, and other services
Preferredqualifications for this position include:
Experience in hospital inpatient pharmacy
