SUMMARY Responsible for management scope and objectives of nursing service in long-term care facility and directs nursing activities by performing the following duties personally or through subordinate supervisors. Has administrative authority, responsibility, and accountability for assuring functions and activities of the nursing services department. Participates in the development of any facility policies that affect the Nursing Services department.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Has knowledge of and ensures that the Nursing Services department adheres to state and federal rules and regulations concerning delivery of resident care.
Is informed regarding residents' conditions, including when significant change in residents' condition warrants nursing or medical intervention.
Responsible for ensuring development and maintenance of recruitment, orientations, in-service education, evaluation, and termination of Nursing Service staff. Demonstrates leadership abilities.
Determines nursing staff's capacity for providing nursing care for any person seeking admission to the facility.
Demonstrates ability to establish priorities and organize effectively. Has excellent written and verbal communication skills.
Assumes responsibility for professional growth and continuing education.
Maintains, interprets, and adheres to facility policy and procedures.
Maintains a quality assurance program for nursing services that includes: infection control audits, safety audits, resident care audits, environmental audits, chart audits, nutritional/weight audits, medication audits, and employee competency audits. -These audits may be delegated to other Nursing Services personnel to perform. -The DNS is aware of facility trends and directs plan of correction of these trends. -Evaluates the effectiveness of the QA program by observing trends. -Attends and directs quarterly QA meetings, in conjunction with Administrator.
Ensures that all medications, treatments, and plans of care are administered as ordered in collaboration with the RN Documentation Nurses.
Responsible for assisting Nursing Department and communicating with state surveyors during annual state survey. -Is in attendance during exit conference. -Assists administration with writing plan of correction.
Evaluates nursing care delivery by making rounds on a daily basis.
Attends resident care planning conferences as needed. -Assists in coordinating provisions of services with other health care providers.
Notifies physicians of needed visits. -Involves Medical Director as necessary.
Interviews Nursing Staff and makes decisions on hiring. -Reviews job descriptions with new employees. -Ensures employee file is complete.
Ensures that supervisory licensed personnel are adequately supervising staff, to ensure that all nursing staff perform their duties in a timely and efficient professional manner.
Ensures all mandatory in services are held along with other in services needed according to facility QA trends.
Ensures adequate nursing services staffing according to state/federal requirements. -Maintains a written staffing plan. -May delegate staffing to another qualified individual. -Supervises staffing plan as performed by other individuals.
Ensures that each employee recognizes legal responsibilities and functions within scope of practice.
Performs and ensures that other nursing staff assist with performing employee performance evaluations.
Performs disciplinary action of nursing personnel and counsels as necessary. -Terminates nursing personnel when necessary.
Provides input to change process and supports CEO and nursing home in process.
Demonstrates professional responsibility of conduct and attendance record.
Assists with decisions concerning resident admission to facility in collaboration with Social Services and administration. -May perform a visit to discharging facility to determine admission status. -Communicates all potential admissions to MDS Coordinator.
Assists in ensuring that adequate supplies for nursing care delivery. -Orders supplies through Purchasing Agent on a monthly basis or as needed.
Develops a nursing services budget in collaboration with the Administrator. -Remains within established budget or communicates additional needs to the administrator.
Keeps work area neat.
Establishes priorities for nursing department and delegates as necessary.
Reports all problems, needs, concerns to the Administrator.
Interacts with all members of the interdisciplinary team in a professional manner.
Interacts with resident families in a professional manner.
Acts as a resource person and accepts guidance from available resources.
Participates in the education of staff, residents, and family members.
Is responsible for meeting own continuing education according to state law. -Attends outside conferences and shares information.
Recognizes legal responsibilities and functions within scope of practice.
Prepares and maintains nursing policy and procedure manual.
Establishes system of records and reports.
Advises management in development and maintenance of facility sanitation and housekeeping practices.
PERIODIC JOB DUTIES Daily: 1) Staff--Make sure all shifts are covered 2) Review potential admissions with ADM/SS 3) Assist Documentation Nurse as a resource regarding assessment for condition changes 4) Meet all admissions--if possible assist nursing with admit procedures 5) Interview potential staff 6) Review all accident and occurrence reports. Investigate and evaluate as needed, instituting a plan of correction as needed. Weekly: 1) Report to Administrator and meet to review week 2) QA audits 3) Meet with staff in stand up meeting, review weekly. 4) Attend Care Planning prn--review chart for at least one resident and sit in on care planning, offering suggestions PRN. 5) Attend other meetings as scheduled--safety, PT, Department Head, etc Monthly: 1) Analyze QA audit info & write reports--in services, study, new policies needed? 2) Staffing schedules 3) Review files for performance evaluations 4) Policy review for quarterly QA meeting 5) Assure adequate nursing supplies Quarterly: 1) QA Meeting 2) New policies to write and implement? Annually: 1) Assist with scheduling of in services for year as needed 2) Prepare for survey 3) Establish yearly schedule for policy review
SUPERVISORY RESPONSIBILITIES Directly manages subordinate supervisors (Documentation Nurses) who supervise an average of 70 employees in the Nursing Department. Is responsible for the overall direction, coordination, and evaluation of this department. Also directly supervises two non-supervisory employees (Medical Records clerk and Nursing Services Coordinator). Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
