The RN in the position of Cancer Case Manager tracks patients to facilitate consultations and appointments in cancer workup, treatment, surveillance and end-of-life care, to include providing intensive case management to cancer patients at greater risk. This group of patients includes those newly diagnosed, in the process of a cancer workup/cancer treatment plan or those patients with a unique issue or barrier to further care (transportation, supportive care, psychosocial issues). The case manager also provides support to physicians on a daily basis: coordinating dual care with community and other VA facilities; ensuring all needed records, tests and other information is available to provider at time of consultation to allow provider to focus on treatment plan and patients questions in an appropriate and timely manner. The Cancer Case Manager also supports the Emergency Department and inpatient units, providing emotional support, education and guidance to patients and families, especially at the time of a newly diagnosed cancer. The incumbent works closely with others VA Medical Centers and community facilities to remove obstacles that would impede the Veteran from utilizing the services of those medical centers not available at our facility. These cases are complicated and need close follow-up. The Cancer Case Manager works closely with the other Cancer Case Managers, providing coverage for planned and unplanned leave. The Cancer Case Manger is expected to collect specific cancer data and report data to facility leadership. The Cancer Case manager will participate in tumor board, cancer care committee, and any specific assigned cancer improvement work groups and/or committees.
