Providence is calling a Clinical Support Program Coordinator (PMG Clinical Support Services, Day Shift, 1.0 FTE) to Providence Medical Group in Missoula, Montana. In this position you will:
Work collaboratively with all providers and staff members, with a focus and emphasis on nursing and medical assistant staff members, to develop clinical support processes that ensure the delivery of efficient, compassionate and high quality care.
Work with a centralized team helping providers, clinic staff and management to fulfill the needs of the Centralized Services department in support of our Patient Centered Medical Home objectives.
Perform duties including chronic disease management, preventative health coordination of services, external result entry, prior authorization of medication and procedures, referral coordination, tracking and managing the referral work queue, prior authorizations or updating patient medical records.
Be responsible for follow up on patient appointments and notes, assuring it is received by the referring provider.
Required qualifications for this position include:
High School graduate
Preferred qualifications for this position include:
Licensed Practical Nurse (LPN)
1 or more years of referral tracking and coordination experience
