The Director, Population Health is primarily responsible for the operational functions of the ACO and include CMS connectivity, CIN requirements, overseeing the A/P and Finance functions of the ACO / CIN, supporting development and implementation of CHRISTUS Health's Clinical Integration/Accountable Care organization (CIN/ACO), working in coordination with the Executive Director Population Health implementation team, and regional leadership. The Director will work with System and Regional leadership in communicating the strategic vision for population health and the CIN/ACO organization, and in supporting implementation of associated strategies and initiatives so they are uniformly deployed across CHRISTUS Health with a focus on: maximization of profitable growth; effective and efficient operations; leveraging of best practices; standardization of systems and practices; driving continuous improvement, innovation, and quality; facilitation/acceleration of population health implementation activities; and, consistency with CHRISTUS' mission and values
The Director will have demonstrated experience in the area of healthcare systems, data analytics, population health management, including clinical integration and accountable care organizations. In addition, the incumbent must demonstrate the following: strong interpersonal and relationship skills; ability to work effectively with physicians and be capable of establishing and maintaining strong communication and relationships with CHRISTUS Health leaders and peers; ability to influence others to achieve key objectives and outcomes; capability of working in a complex, matrixed organizational structure with multiple constituencies in various markets with differing issues and demands; ability to assess and mediate complex issues and articulate solutions; commitment to strongly and consistently promote CHRISTUS Health's population health vision and strategies; ability to make difficult decisions and take action; and, serve as a resource to CHRISTUS Health in the field of population health.
Develops and implements clinical integration strategies, including ACO and CIN initiatives, working with system and regional leadership and physicians
Develops and maintains strong relationships with physician leaders, system staff and peers to influence decisions and to achieve the strategic vision for population health
Pursues opportunities for partnerships, collaborations and new programmatic opportunities that may offer enhancements
Promotes population health initiatives through education of staff and physicians
Works closely with the system population health team to develop strategic and implementation plans for population health initiatives
Promotes implementation of standardized processes and systems in each CHRISTUS region to fully maximize the value and profitability of population health activities
Facilitates completion of activities and projects necessary to advance Population Health goals.
Promotes transparency and public reporting of clinical data and new reimbursement incentives.
Ensures compliance with all relevant legislative and regulatory requirements.
Strategic Planning and Marketing
Works with the Executive Director, system population health team and regional leadership in developing, implementing and monitoring the strategic and implementation plans for population health
Works with the ACO/CIN Boards, senior leadership, and physicians, in developing and annually updating the participant's strategic goals.
Implements effective provider beneficiary and communication strategies consistent with established direction
Provides leadership in implementing physician integration initiatives, ensuring compliance with applicable laws, rules and standards of all regulatory bodies and accreditation agencies.
Oversees key operational initiatives, efficiencies and structure, and identifies priorities that enhance overall performance
Provide input into annual budget process consistent with departmental goals.
Monitors financial operations to ensure that the organization utilizes its resources in a fiscally responsible manner.
Maintains timely communication with the Population Health Department, regional leadership, fellow Associates, physicians, and the ACO and CIN Boards of Directors
Supports the organizational and management structure such that it achieves established goals and objectives.
Legal and Governance
Works in concert with the ACO and CIN Board(s) to ensure that the Boards are fully informed and educated on areas related to the success of the organization
Works closely with Board chair in developing board meeting agendas, and in helping to conduct meetings of the Board
Supports policies and procedures necessary for compliance with all healthcare legal and regulatory requirements.
Education and Training:
Bachelors degree in Health Administration or other business-related degree. (MHA, MBA, MPH or equivalent preferred)
Minimum of three (3) years working experience in an acute care hospital or health system.
Demonstrates interest and involvement in American College of Healthcare Executives as a member or a fellow.
Strong financial and process oriented background
Excellent verbal and written communication skills to communicate with a variety of individuals including physicians, the general public, volunteers, and associates
Ability to motivate others
Ability to work in a rapidly changing environment
The ability to interact with CHRISTUS Health leaders, ACO/CIN, board members, physicians, community and business leaders as necessary
Must have strong understanding of and demonstrate skill in leadership, strategic management, change management, communications, team building, continuous quality improvement, problem-solving, decision-making, innovation, financial management of operations, and stewardship of resources
Skill in establishing a balanced perspective on mission effectiveness and business results is critical.
