Provides assistance to the Director of Pain Management Service and Pain Management attending physicians. Assures a smoothly coordinated effort in instituting given departmental policies. Identifies, communicates and assists in the solution of patient and departmental problems.
Assists with patient care and management in the Pain Management Clinic, Inpatient Units and Operating Room, under the responsibility and supervision of the attending physician. Performs diagnostic and therapeutic tasks to allow the attending physician to extend services.
Provides careful and thorough gathering of the data required to identify, select and implement the diagnosis and therapeutic plan with the attending physician.
Evaluates functional outcomes as they relate to Pain Management. Understands the characteristics of chronic pain patients. Deals with associated psychological and social problems including depression, suicidal ideation, marital and other relationship problems, disability and related social, legal and financial problems, in order to enable a successful treatment outcome.
MAJOR DUTIES AND RESPONSIBILITIES:
-Independently provides patient assessment. Coordinates patient care and provides patient education and follow-up and triage complex patient to ensure optimal scheduling.
-Obtains history and performs physical examinations on all new patients including pain level, psycho-social-behavioral factors, medical conditions and rehabilitation status. Formulates a working diagnosis for treatment.
-Provides assessment of return (follow-up) patients to promote longitudinal continuation of care.
-Writes routine admission orders and arranges appropriate lab and x-ray studies. Initiates and coordinates consults with other treating services including medical, psychological, social, orthopedic, oncology, neurosurgery, rehabilitation, and patient and financial aid services.
-Provides patient education regarding diagnosis, procedures, risk and benefits, outcomes and follow-up treatments.
-Takes calls and performs on-call functions for all admitted Chronic Pain Management patients, including patients being treated by Pain Management on other admitting services.
-Completes records, inpatient care, discharge summaries and letters to referring physicians in a timely manner.
-Provides patient teaching at discharge, as well as coordination of follow-up care.
-Researches and develops policies and procedures, treatment guidelines and protocols, and educational materials for the care of a diverse group of patients. Participates in quality improvement (QI) program measures, new procedure development and technical instruction.
-Teaches new employees, residents, visiting physicians, medical and nursing students, allied health providers, etc., in the function and utilization of Physician Assistants related to patient care in the Pain Management Service (Inpatient and Outpatient settings).
-Maintains DEA registration, required CME hours, national board certification and registration status.
-Participates in appropriate departmental and institutional meetings including Morbidity and Mortality Conference presentations and discussions, as well as teaching presentations to other providers and/or trainees.
-Assists the Pain Management (Attending) Physician in delivering the highest quality of care to improve positive patient outcomes.
-Assists the Attending Physician in performing therapeutic procedures in the clinic, Operating Room, and Inpatient unit, based on Physician Assistant's training and experience in pain management.
-Assists the Attending Physician in scheduling and interpretation of requested x-rays, MRI, CT, EMG and other tests that are required to develop working diagnosis and treatment plan.
-Makes rounds on inpatients with the Pain Management Physician, or independently (reporting back to the Pain Management Physician), and writes pertinent notes on charts of Pain Management patients.
-Performs as First Assistant at surgical procedures in the Operating Room. Performs wound closures under the direction of the Attending Physician. Operates the radio-frequency machine and other surgical equipment, as needed, for the Attending Physician during procedures.
-Coordinates daily Inpatient care activities and requirements with the daily (On Call) Attending Physician.
-Coordinates pump patients' care by initial assessment of pump trial medication dosing, as reviewed, approved and ordered by attending physician.
-Contacts pump implant patients to assess needs for future treatments, including refill of pump Medications and other medications, in consultation with the Attending Physician.
COMPETENCIES AND SKILLS:
-Qualified as a surgical first assist.
- Demonstrates ability to work in a team environment, compassion and genuine interest in caring for patients experiencing chronic pain, proficient and efficient data entry using the electronic medical record and the ability to deal with patients in such a way as to instill confidence and ease of communication.
- Demonstrates ability to work with patients with cancer, chronic pain and acute pain, including social, financial and psychological components.
- Demonstrates experience with, and ability to recognize, counsel and refer patients exhibiting behavior suggestive of tolerance, pseudo addiction and addition to appropriate medical management teams and detoxification programs.
- Demonstrates ability to participate in growth and development of the department, including ability to recognize and improve patient flow, efficiency, procedures, policies etc, to enhance and ensure patient and staff satisfaction.
- Demonstrates understanding of Insurance allowances and limitations and revenue cycle elements.
- Completes in-house annual education/training requirements, as delineated by Joint Commission, DOH, etc, required.
-Experienced and computer-literate, with knowledge and experience in medical information and patient billing systems, required. WORKING CONDITIONS: - Work is typically performed in an Outpatient, or Inpatient environment. - Work may be associated with stress in caring for complex pain patients experiencing social, psychological, and personal problems. - Job will include occasional patient lifting and/or assisting co-workers with patient lifting. - May require prolonged periods of standing - Travel as indicated - Participation in the holiday/weekend/ and call schedule as indicated
EDUCATION AND/OR EXPERIENCE:
-The successful masters prepared Nurse Practitioner candidate will work in collaboration with the physician and have a certificate of completion from an approved program for Nurse Practitioners or be eligible to hold national certification that allows the NP to work with the patient populations and location settings associated with this position. Must be eligible for licensure as a nurse practitioner in the state of PA.
-The successful Physician Assistant candidate will work under the responsibility and supervision of the physician and require completion of an ARC-PA (or predecessor organization) approved physician assistant program, current NCCPA certification or eligibility and Pennsylvania State licensure. -- --
-Three years of experience preferred. -DEA license required. -Excellent computer skills including EMR experience (EPIC usage preferred)
