The Housing Specialist works under the supervision of the Housing Coordinator to support the provision of care coordination services for Patients who are experiencing homelessness or housing instability as they transition to stable housing. Services are offered in a community-based setting (e.g. ACHN clinic, coffee shop) or where the Patient is living, inclusive of in a shelter or places not meant for human habitation (e.g. park, tent, bus, train). The Housing Specialist helps Patients navigate the health care delivery system, advocates on their behalf and works to reduce barriers that interfere with Patients' ability to successfully interface with their medical home provider. The Housing Specialist supports Patients experiencing homelessness or housing instability access existing funding streams and housing programs to gain stable housing so to address a key Social Determinant of Health. This is a grant funded position.
This position is exempt from Career Service under the CCH Personnel Rules.
MINI MUM QUALIFICATIONS:
Bachelor's Degree from an accredited college or university (Must provide official transcripts at time of interview)
One (1) year of work experience with culturally diverse population
One (1) year of work experience with persons experiencing homelessness or housing instability
Prior experience assisting people with placement into housing programs with varied regulatory requirements (e.g. Section 811, IHDA RHSP)
Prior experience performing health risk screenings, providing health education or supporting linkages to community based resources for a health care organization or community based social service agency
Must possess a valid driver's license and insured vehicle or otherwise provide an acceptable and reliable means of transportation
Proficient in Microsoft Office (Word, Excel, PowerPoint) and scheduling software
Graduate of a Community Health Worker (CHW) Program
Knowledge, Skills, Abilities and Other Characteristics:
* Excellent verbal and written communication skills necessary to communicate with all levels and staff and a Patient population composed of diverse cultures and age groups
Ability to track meetings, appointments and e-mails in Microsoft Outlook
Ability to effectively prioritize work
Ability to meet deadlines
