This position is for a Registered Nurse (RN) Clinical Transfer Coordinator for the Chief of Staff service line at the Richmond VA Medical Center. The Coordinator is directly responsible to the Medical Center Chief of Staff. This position collaborates with the interdisciplinary health care team, community agencies/facilities, and patient/families/significant others to ensure continuity of care across all levels of care insuring timely placement of VA patients into the appropriate level of care.
