Work Hours/Shift:

• Full-Time, Monday – Friday

You Will Be Responsible For:

• Successfully applies strategic thinking on an ongoing basis, to help maintain current state as well as develop future state. Maintains up-to-date clinical knowledge and applies that in the development of future state processes, as well as in the analysis of current state processes. Completes assignments in a timely and cost-efficient manner.
• Utilizes visionary and strategic thinking on all assignments, including the training and any applicable change management processes.
• Completes documentation for all phases of assignments as appropriate.  Effectively prepares and presents technical and/or educational documentation of assigned projects and/or systems to enhance the knowledge of affected individuals.
• Assists with operational impacts of electronic and/or operational solutions, performs ad hoc audits to assess performance and provides recommendations for future education needs or process change, with an ongoing focus on world-class, global pace-setting performance.
• Participates in and conducts internal and/or external meetings and training programs, staying current and compliant on key regulatory and/or statutory issues that may affect current/future assignments, informing leadership of these issues and related impacts
• Stays up to date on emerging best practices and research related to health management approaches in value- based care environments.
• Performs other duties as assigned or directed to ensure smooth and efficient operations of the Health Management team. Automatically looks for extra work as workload permits.   

What You Will Need:

Education and Experience Required:

• Bachelor’s Degree in health care related area
• Extensive experience with Microsoft Office suite, including Excel, PowerPoint, Outlook, and Word
• At least 3 years’ experience in health plan or ambulatory care/case management setting
• Experience in developing educational content for clinical staff

Licensure, Certification or Registration Required:

• If a nurse, requires Florida licensure as registered nurse (RN)

Knowledge and Skills Required:

• Ability to research current health care trends and best practices in the area of health management and value-based care, and synthesize/compile those recommendations to inform strategic planning for the future of health management
• Excellent verbal and written communication skills, with a focus on strategic thought and development.
• Ability to assist in ongoing performance audits and oversight to maintain current accreditation
• Excellent ability in making educational presentations to management and staff
• Ability to translate business need into practice, determine accuracy of information provided and deliver results to meet the intended request
• Comfortable in a complex and changing healthcare environment.
• Self-motivated requiring minimal management supervision. Able to multitask.

Job Summary:

The Care Integration Strategist serves the PHSO Health Management Team system-wide by providing accurate and timely research, strategies, education and execution of approved assignments, to help support our goal of managing population risk. Works and communicates under general supervision of the PHSO Care Integration Manager. The Care Integration Strategist assists in the planning, training, implementation, and re-evaluation of best practice processes related to the managed risk imperative. In addition, The Care Integration Strategist is actively involved in policy development, program development, ongoing audits and quality improvement efforts. Consistently communicates and reports assignment status to management and involved team members, tracks assignments against proposed timelines, updates plans to reflect current progress, and escalates issues in need of further attention the appropriate area with detailed and accurate information. The Care Integration Strategist actively participates in assuring outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
