Term appointments are non-status, nonpermanent appointments of limited duration (more than 1 year but not more than 4). The Licensed Practical Nurse works under the direction of a RN and reports to the Primary care Nurse Manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the implementation of the patient-driven holistic care plan for life. The LPN is a member of the clinical care team. The LPN demonstrates active collaboration and communication with the Patient Aligned Care Team (Patient, Provider, RN Care Manager and Medical Support Assistant) as well as expanded team (family/caregiver, internal and community based services involved in providing care to the patient). a. Provides support for the Patient Aligned Care Team in the Primary Care clinics following established nursing and primary care policies and procedures. b. Demonstrates age appropriate care for the following age groups whom may be experiencing medical and psychological consequences of illness: adult, age 18, to the elderly adult, age 65 and over. c. Provides a safe environment as demonstrated by compliance with sound safety practices following VA CWM HCS regulations. d. Participates in selected health screenings, clinical reminders, telephone calls and nursing treatments as assigned. e. Administers medications according to the 5 rights of Medication administration. Documents medication administration using BCMA/CPRS. Follows policy for administration of controlled substances. Accesses and utilizes Unapproved Abbreviation List. f. Ability to draw, centrifuge and package lab samples when indicated. g. Demonstrates proper infection control techniques including but not limited to: hand washing, blood borne pathogens, needle stick management, and personal protective equipment. h. Demonstrates ability to effectively handle multiple projects and duties simultaneously; prioritizes appropriately. Work Schedule: The work schedule will be determined based on the needs of the Medical Center. The New England VA Medical Centers are acute care facility operating 24 hours a day 7 days a week. While most tours of duty are scheduled during the day, the possibility of working evenings, nights and weekends to exist.
