Participates in departmental policy, procedure and performance improvement development. Encourages staff participation project development, committees and educational offerings. Offers support and expertise to the nursing staff, patients, and families. Serves as a resource and assists in problem solving. Ensures the provision of education and teaching by Home Care staff to patients and their families. Investigates variances and patient dissatisfaction reports. Takes appropriate action in coordination with the Home Health Clinical Practices Manager. Reviews orders, notes, and care plans for appropriateness. Reviews closed and open charts to maintain on going quality assurance. Reviews and updates new admissions and recertifications per policy and procedure. Participates in PI activities. Assists in completing insurance verification information and ongoing authorizations. Promotes open communication between nursing and therapy disciplines. Plans and conducts staff and RN meetings, as needed. Coordinates patient visits with the various disciplines. Evaluates and assists with development of forms for use in the documentation of Home Care service provision. Knowledgeable of OASIS guidelines and home health coding standards. Reviews and educations staff on a routine and regular basis. Knowledgeable of PPS and SHP reporting. Immediately researches any issues.
Identifies learning needs of the nursing staff. Assists with organizing and providing for in- services and preceptor programs by utilizing available resources. Evaluates results. Works to assist staff to meet the required and identified competencies and skills necessary to nursing staff, and takes reasonable steps to ensure staff in-service requirements are met. Observes, educates, and precepts staff on performance of procedures and assures compliance of departmental policy and procedure. Assesses the teaching protocols for the high risk diagnoses and for pre and postoperative teaching. Collaborates with Home Health Manager of Clinical Practice regarding discipline of employees. Responsible for developing, implementing and monitoring employee work improvement plans. Collaborates with the Home Health Manager of Clinical Practices in completing department quality assurance and performance improvement programs that meets hospital guidelines. Collaborates with Home Health Manager of Clinical Practices in reviewing incident reports/customer concerns and takes appropriate action and makes recommendations to minimize future occurrences and monitors follow up. Collaborates with Home Health Manager of Clinical Practices to assist and/or conduct monthly departmental staff meetings for all assigned employees, adequately covers all current items of business, PI, education, information sharing with staff and actively listens to staff input. Promotes interdepartmental relationships by utilizing appropriate communication skills and problem solving techniques. Makes recommendations for the selection of staff members for special assignments to the Clinical Manager. Makes recommendations for the purchase of materials or equipment for the improvement of nursing care to the Home Health Manager of Clinical Practices. Promotes positive, constructive interactions between staff members. Evaluates the morale of the staff in relation to job satisfaction and interpersonal relationships and reports concerns to the Clinical Manager. Collaborates with the Home Health Manager of Clinical Practices in effective hiring, recruiting, assigning and scheduling staff. Completes all performance appraisals on direct reports by due date. Effectively supervises all assigned staff in the performance of their daily work activities. Effectively deals with under performing employees with collaboration from Home Health Manager of Clinical Practices - communicates clear expectations, consults with Human Resources as appropriate. Minimum Education Requirement
a. Graduate of an accredited school of nursing. BSN desired. b. Minimum Experience Requirement Minimum three years experience as a registered nurse with a minimum of two (2) year experience in home health, hospice or related setting required c. Minimum License Requirement Current license as a registered nurse in good standing for the State of South Carolina. Current BLS Certification prior to having patient care. Current SC driver's license, valid car insurance and must maintain a driving record that is insurable under AAMC's auto insurance policy.
