You Will Be Responsible For:
Issue and order Controlled substances and maintain records as required by State and Federal Law
Compound and dispense Pharmaceuticals, including Chemotherapy in accordance with USP and Board of Pharmacy Standards
Respond to and follow up on daily Pharmacotherapy consults
Carry out daily requirements for anticoagulation management, Renal dosage management, TPN formulation management, Antibiotic LOT management, Formulary substitution management, daily monitoring of all pertinent Laboratory data necessary for drug specific findings, Pharmacokinetic dosing of medications
Monitor Provider order entry for Interactions, over/under dosing, contraindications, pregnancy and lactation, allergies and appropriateness for therapy.
Documentation of adverse drug reactions, all clinical activities and interventions and consultations
Training of Pharmacists, nurses, Technicians and other staff as requested.
Participation in formulary review and Hospital committees as requested
What You Will Need:
BS in Pharmacy from accredited school
Minimum one year of Hospital Pharmacy experience
Current State of Florida Pharmacist License
Clinical Pharmacist are responsible for verification of all appropriate provider orders, insuring the availability of all necessary medications, carrying out all duties associated with the FHNP Clinical Pharmacy Program and oversight of all Technician/Buyer activities and duties.
