The VA Maryland Health Care System (VAMHCS) is a Level I tertiary multi campus teaching facility affiliated with the University of Maryland, School of Medicine and over 100 educational institutions. The VAMHCS consists of the acute care Baltimore facility; the multi building Perry Point campus ; the Loch Raven campus ; and six urban and rural outpatient clinics. The Patient Aligned Care Team (PACT, also known as the Medical Home Model) LPN is a vital team member and the first clinician to contact with the Veteran at their Primary Care Clinic appointment. The LPN checks vital signs, obtains patient's weight, interviews the patient for clinical reminders, other documentation and administers injections. The LPN maintains close communication with the Primary Care Provider, PACT RN, and administrative associate. Duties also include point-of-care testing for glucose, hemoglobin A1C and PT/INR, phlebotomy as well as performing 12-lead EKG as needed. The LPN serves as patient advocate educating patients on health promotion and disease prevention related to regarding immunizations and follow up for chronic disease management.
