The Nurse Practitioner provides primary care services to established panels, adult through geriatric, Veteran patients and functions as a member of a patient aligned care team (PACT) consisting of an RN Care Manager, a clinical associate (LPN or Health Tech/Medical Assistant), and a clerical associate (Medical Support Assistant). The PACT model provides comprehensive interdisciplinary care in coordination with extended support (e.g. pharmacist, social worker, dietician, psychologist, etc.). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
