The Registered Nurse (RN) - Specialty Clinic/PACT demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient driven holistic care through active collaboration, communication, and coordination of resources. The Registered Nurse works in partnership with the assigned primary care provider and a panel of patients for effective disease and chronic care management. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
