The APRN/PA is a member of the CLC team and is expected to conduct comprehensive evaluations and management of veterans with a wide variety of medical and psychological problems, including veterans at the end of life. The APRN/PA is expected to review medical records, to complete patient/caregiver interview, complete comprehensive geriatric assessments, screen, and address geriatric syndromes, to address Life Sustaining Treatment Orders, to create and implement comprehensive treatment plans..
